The association between antenatal care utilisation and women’s empowerment in Nepal: A secondary analysis of the 2022 Nepal Demographic and Health Survey

ABSTRACT
BACKGROUND:
Antenatal care (ANC) is a critically important maternal and child health service that prevents maternal mortality. To encourage the utilisation of ANC, women’s empowerment like autonomy and decision-making ability have been reported to be associated with attending four or more (≥4) ANC contacts. However, theoretical framework has not been fully adopted and comprehensively analysed the association between ANC utilisation and women’s empowerment by using the most recent data from Nepal.

OBJECTIVES:
This research aims to examine the association between the number of ANC contacts and women’s empowerment among Nepali women.

METHODS:
This research was a secondary analysis using the 2022 Nepal Demographic and Health Survey. The data of women aged 15-49 years who had given birth or experienced stillbirth in the past two years and lived with husband or partner and were included. The dependent variables were the number of ANC contacts, the independent variables were women’s empowerment variables, and the covariates were socio-demographic characteristics. Chi-square tests were conducted to examine statistically significant associations between the number of ANC contacts (0–3 vs. ≥4 contacts, 0–7 vs. ≥8 contacts) and independent variables or covariates. Subsequently, multiple logistic regression analysis was performed with adjustment for covariates.

RESULTS:
Total of 2,012 women aged 15-49 were analysed. 80.2% attended ≥4 ANC contacts, while only 5.8% attended ≥8 ANC contacts. Bivariate analysis revealed the statistically significant association between the number of ANC contacts and some variables such as women’s educational level, Internet use and bank account ownership.Multivariate analysis revealed that women who used the Internet almost every day were 2.13 times more likely to attend ≥4 ANC contacts compared to non-users (95% Confidence Interval: 1.44–3.15, p<.001).

CONCLUSIONS:
The ANC utilisation may be associated with several factors such as Internet use.
