Transcultural adaptation and psychometric validation of the Iranian version of the quality of respectful maternity care questionnaire for the Portuguese context of labor care

ABSTRACT
BACKGROUND:
The Quality of Respectful Maternity Care Questionnaire in Iran (QRMCQI) assesses maternal perceptions of the care received during labor, but it is culturally adapted and validated for Iranian women. The absence of validated instruments in European Portuguese to assess the quality of respectful maternity care during labor motivated this research.
OBJECTIVES:
To cross-culturally adapt and assess the psychometric qualities of the QRMCQI in Portuguese women.
METHODS:
This is a cross-sectional methodological study, conducted according to ethical principles, which included several phases: translation and cultural adaptation of the original instrument from Farsi to Portuguese, with the author's permission and support from experts; content validation by experts in maternal health and education; assessment of apparent validity with women from the target population; and a pilot test. Psychometric validation included confirmatory factor analysis and reliability assessment.
RESULTS:
Initially, the Content Validity Ratio (CVR) applied to the 60 items in the questionnaire indicated that 10 did not meet the minimum criterion and were eliminated. In calculating the Item-Content Validity Index (I-CVI), two items had values ≤ 0.78 and were also removed. Test-retest reliability analysis, with 200 participants, demonstrated excellent stability, with weighted kappa coefficient greater than 0.81 for most items. Confirmatory factor analysis revealed a good fit of the model (χ²/df=1.94; CFI=0.988; TLI=0.987; GFI=0.981; RMSEA=0.029; SRMR=0.061), resulting in a final version with 47 items. Overall reliability, measured by McDonald's ω coefficient, was 0.910, indicating high internal consistency.
CONCLUSIONS:
The Portuguese version of the QRMCQI has robust psychometric properties and is a valid and reliable instrument for assessing experiences of maternity care in hospital births.

Its application can support research and guide continuous improvement programs in institutional maternal care.
