ABSTRACT
BACKGROUND:
Nigeria is burdened with 20% of global maternal mortality. Antenatal care connects women to facility birth and responding to life threatening morbidity. Group ANC is an innovative women centered model found to improve quality and satisfaction through peer support. Project HOPE with the Ministry of Health commenced G-ANC in Niger state for 6 months from September 2022 to March 2023.A baseline assessment found critical gaps in midwives, essential equipment, and 24/7 care availability.
OBJECTIVES:
To document the implementation process, and G-ANC impact in a very low resourced setting, including essential steps for training and enabling environments, service utilization changes, and health outcomes.
METHODS:
A multivariate interrupted time series (ITS) analysis using linear regression was conducted which analyzed 8 variables: ANC 1 and 4, births and proportion of live births, malaria positivity in febrile patients, post-partum contraception to evaluate changes in service utilization and maternal and newborn health indicators following an intervention.
RESULTS:
Three hundred health care workers and managers were trained on G-ANC, two for every health care facility. G-ANC was established in 150, or just over half of the primary health facilities. Over 17,700 women participated in 1054 antenatal care groups. Due to security risks, some health care providers moved to safer facilities for training, and one PHC was closed. ANC 1 and ANC 4, facility births, and live births, had a statistically significant effect of time (p < 0.001), level (p = 0.027) or slope (p < 0.001) after the initiation of the intervention.
CONCLUSIONS:
A brief G-ANC intervention increased service utilization and improved perinatal outcomes. This can be attributed to the strength of G ANC, and the importance of synergizing all partners to maximize impact.
KEY MESSAGE:
G-ANC when effectively implemented can have impact on live births even with significant challenges including inadequate midwives. More research is needed.
