The effect of acupoint hot compress on reducing labor pain in primiparous women: A randomized clinical trial

ABSTRACT
BACKGROUND:
Acupoint hot compress may be a safe, convenient and cost-effective non-drug analgesic intervention, but there is limited evidence at present.

OBJECTIVES:
To assess the effects of acupoint hot compress on the labor pain, anxiety and fear of childbirth and sense of delivery control in primiparous women.

METHODS:
A total of 186 primiparous women at 37-42 weeks of gestation who were admitted to our hospital for vaginal delivery were randomly assigned to a control group and an acupoint hot compress group. The acupoint hot compress group received routine delivery care plus acupoint hot compress to the Baliao acupoint, while the control group received only routine delivery care without acupoint hot compress. To evaluate labor pain, anxiety and fear of childbirth, sense of delivery control in primiparous women, we use the Visual Analog Scale (VAS), State Anxiety Inventory (S-AI), Wijma Delivery Expectation Scale (W-DEQ-A), Wijma Delivery Experience Scale (W-DEQ-B), and Labour Agentry Scale(LAS).

RESULTS:
The intervention group received Baliao acupoint hot compress for an average duration of 230.7 minutes. Compared to the control group, the acupoint hot compress group had significantly lower VAS scores at 3h and 4h after regular labor (p=0.013 and p<0.001) and in 3cm cervical dilatation (p<0.001), as well as lower fear of childbirth (p=0.012), higher sense of delivery control (p=0.004) and shorter first stage of labor (p=0.039). However, no statistically significant differences were observed between the two groups regarding anxiety of childbirth, intraspinal analgesic administration and delivery outcomes.

CONCLUSIONS:
The application of acupoint hot compress can markedly alleviate labor pain during the latent phase, diminish fear of childbirth, improve the perception of control during delivery, and facilitate advancements in the initial stage of labor. This method has been validated as an effective and safe non-pharmacological approach to pain management for primiparous women undergoing natural labor.
