ABSTRACT
PURPOSE:
Implementing and evaluating My Midwife Malmö, a Midwifery Continuity of Care (MCoC) model for women with vulnerabilities during pregnancy, birth and the early postnatal period.

DISCUSSION:
In Sweden, standard care during pregnancy, birth and the postnatal period is fragmented, and the centralization further restricts the availability of MCoC. Within My Midwife, approximately 300 women in Malmö are currently given the opportunity to receive MCoC each year. My Midwife is available for women with vulnerabilities such as fear of birth, mental health issues, and when facing language barriers. My midwife ensures relational continuity of care with availability around the clock. A primary midwife surrounded by a small team of midwives provides care during pregnancy, birth, and the early postnatal period. The model of care includes antenatal check-ups during pregnancy and offers home-based care during late pregnancy and the latent phase. During labour the women receive one-to-one care, and the midwives also provide home-based care in the early postnatal period.

EVIDENCE WHERE RELEVANT:
Several studies have demonstrated positive effects of MCoC with a reduced need for medical interventions during labour and birth, and improved outcomes for women and their babies. Existing research on MCoC in Sweden suggests potential benefits, but the evidence is limited. Notably, there is a significant knowledge gap regarding the provision of targeted care for vulnerable women within a structured continuity of care model. Evaluation of women's experiences and medical outcomes within My Midwife Malmö is currently being conducted.

KEY MESSAGE:
The implementation of My Midwife has proven feasible, encompassing antenatal check-ups, one-to-one care during labour and birth, as well as home-based care during late pregnancy, the latent phase, and the postnatal period. Preliminary results show that women receiving care through My Midwife report increased feelings of participation, safety, accessibility, and overall satisfaction compared to standard care.
