ABSTRACT
PURPOSE:
This proposal presents a midwife-led sexual and reproductive health (SRH) care model for migrant, displaced, and refugee women along Mexico’s northern border, with particular emphasis on survivors of sexual violence.
DISCUSSION:
Mexico’s northern border, particularly Baja California, faces a humanitarian crisis due to the growing number of migrants and asylum seekers. Many of these women experience sexual violence during their journey or upon arrival, facing limited access to essential SRH services. Midwives play a key role in providing culturally appropriate, community-based, and rights-focused SRH care. The model is already implemented in Baja California, positioning midwives as essential actors in the humanitarian SRH response, coordinating with health services, shelters, social organizations, and human rights groups.
EVIDENCE WHERE RELEVANT:
In 2023, over 25,000 asylum seekers were reported in Baja California. Research shows migrant women face a 4.5 times higher risk of sexual violence than male migrants, especially along high-risk routes such as La Bestia, shelters, and border crossings. Transactional and survival sex are common strategies to meet basic needs or avoid violence. Restrictive migration policies further limit access to health services and protection, deepening health inequities.
The care model includes:
-Trauma-informed, person-centered care addressing physical, mental, emotional, and spiritual health.
-Creation of safe spaces and dignified care for survivors of violence.
-Reduced barriers to SRH services.
-Comprehensive care for pregnancy, childbirth, postpartum, safe abortion, menstrual and mental health.
-Strengthened midwifery networks and coordination across sectors.
KEY MESSAGE:
Midwives are essential to guaranteeing SRH services for migrant women, reducing maternal morbidity and mortality, and ensuring access to safe abortion. This model reduces health inequities, improves women’s well-being during transit and at their destinations, and promotes a more equitable distribution of reproductive health care tasks.
