ABSTRACT
PURPOSE:
The quality, safety, and humanity of care for mothers and newborns are improved when the Midwifery Model of Care is incorporated into clinical practice. It minimizes unnecessary interventions, encourages healthy, natural birth experiences, and focuses on respect, autonomy, and shared decision-making. It increases trust, enhances communication, lowers costs through preventive care, and promotes cooperation among medical professionals.
DISCUSSION:
The Midwifery Model of Care represents holistic, woman-centered healthcare, addressing physical, emotional, and social needs throughout pregnancy, childbirth, and postpartum. Since 2008, Dhulikhel Hospital, Kathmandu University Hospital, has provided nurse-led antenatal care for low-risk women. Traditionally, the educational pathway for maternity healthcare professionals in Nepal has been through a nursing degree. In 2016, the introduction of the Bachelor in Midwifery program, supported by internationally trained midwives and training in midwifery care, further boosted our confidence to teach students and integrate some elements of this model into practice. To fully implement the Midwifery Model of Care, we established the Onsite Midwife-Led Birthing Unit in May 2022, addressing the urgent need for dignified and respectful maternity care during childbirth. Today, the midwife-led ANC and OMBU together exemplify the Midwifery Model of Care, ensuring safe, positive birth experiences for low-risk women. This setting also supports clinical placement for midwifery students and serves as a model for institutions across Nepal.
EVIDENCE WHERE RELEVANT:
Research indicates that the Midwifery Model of Care improves maternal experiences, such as increased satisfaction and decreased anxiety, and results in more spontaneous vaginal births with fewer interventions. It is affordable and improves access for underserved populations by providing ongoing, culturally competent care. Ultimately, it improves outcomes for both mothers and newborns.
