Developing consensus-based midwifery care recommendations for women experiencing pregnancy loss: A Delphi study with midwifery and perinatal experts in Portugal

ABSTRACT
BACKGROUND:
Pregnancy loss is a profound experience that demands sensitive, respectful, and evidence-based care. While the literature provides insights into women’s needs, professional consensus on specific nursing interventions remains limited, particularly within the Portuguese context.
OBJECTIVES:
To develop and validate a nursing intervention programme for women and families experiencing pregnancy loss, based on expert consensus among Portuguese midwives.
METHODS:
A modified e-Delphi study was conducted in two rounds between February and April 2025. Fifty-two midwives were invited; 37 participated in Round 1 and 33 in Round 2. Inclusion criteria required specialization in maternal health nursing and clinical, academic or research experience in perinatal bereavement. In Round 1, 70 interventions were rated using a 5-point Likert scale. Two interventions were excluded for not meeting the ≥75% agreement threshold. In Round 2, 21 revised or new interventions were evaluated. All met the inclusion criteria. A total of 69 validated interventions were organized into five components.
RESULTS:
The five components reflect the full continuum of care: a) Education and training of professionals – ethical, practical, and emotional preparation. b) Creation of supportive environments for grieving – physical space, privacy, and symbolic recognition. c) Personalized support to the woman and family – emotional, communicational, informational, and symbolic care respecting preferences. d) Post-discharge follow-up – structured, sensitive outreach tailored to the woman's wishes and future pregnancies. e) Emotional and organisational support for professionals – psychological support, debriefing, and institutional strategies to foster well-being.
CONCLUSIONS:
This study produced a rigorously validated, consensus-based nursing intervention programme for pregnancy loss care. It supports the delivery of compassionate, woman-centred, and ethically grounded midwifery practice. The final set of 69 recommendations may inform clinical guidelines, training, and maternity care policies.
KEY MESSAGE:
This Delphi-based intervention programme provides a validated, comprehensive framework to support respectful, person-centred nursing care for women experiencing pregnancy loss across all stages of care.
