ABSTRACT
PURPOSE:
This presentation highlights the intersection of midwifery, evidence-based public health, and political action aimed at improving access to safe abortion care across borders. It focuses on the Dutch parliamentary debate held on April 8, 2024, concerning the use of telemedicine to provide abortion pills in early pregnancy for individuals in countries where legal and structural barriers severely limit access.
DISCUSSION:
Initiated by Dutch MP and midwife Elke Slagt-Tichelman, the debate responds to appeals from feminist organizations such as Women Help Women and Abortion Without Borders. It explores how healthcare professionals and policymakers can collaborate to reduce health inequalities and uphold reproductive rights. Dutch lawmakers are now considering ways to support medical abortion care transnationally, offering a solidarity-based response to restrictive national laws. This includes safeguarding medical standards by addressing contraindications and ensuring that abortions are voluntary and informed.
EVIDENCE WHERE RELEVANT:
Scientific research and WHO guidelines confirm that telemedicine for abortion is safe, effective, and essential where access is limited. The practice of mailing abortion pills after an online medical consultation is well-supported by clinical evidence and contributes to timely, safe, and accessible care.
KEY MESSAGE:
Healthcare professionals have a vital role that extends beyond clinical care—they are advocates for reproductive rights and drivers of systemic change. In alignment with global health evidence, midwives and legislators must work together to expand access to safe and respectful abortion services, including telemedical solutions. Where access is denied, international solidarity can save lives.
