Conceptual analysis of the well-being of mothers raising multiples

ABSTRACT
PURPOSE:
The purpose of this theme is to clarify how the well-being of mothers of multiples(twins,triplets,etc.) is conceptualized in previous studies. It also aims to examine the extent to which existing psychological and health-related scales appropriately reflect the unique physical, psychological, and social experiences of these mothers. By identifying the components and context-specific characteristics of well-being in multiple birth parenting, this theme seeks to provide a foundation for future scale development and support measures.
DISCUSSION:
Well-being is increasingly recognized as an important public health issue worldwide, as demonstrated by the SDGs and WHO policies. Interest in maternal well-being is also growing, and in Japan, the importance of supporting psychological health during the child-rearing period is evident in conjunction with measures to address the declining birthrate. However, the unique needs of mothers raising multiples have not yet been fully understood. These mothers face complex and persistent psychological difficulties, such as feelings of isolation, excessive childcare burdens, and conflicts regarding fairness, while also experiencing positive emotions such as joy, a sense of accomplishment, and family unity that are unique to raising multiples. Their well-being is difficult to fully capture using conventional indicators alone.
EVIDENCE WHERE RELEVANT:
While there have been conceptual analyses of maternal well-being in previous studies, there are no studies that systematically analyze the well-being of mothers of multiple births. The WHO's Geneva Charter (2021) and well-being promotion framework indicate the need for context-appropriate, equity-focused tool development. This theme addresses this issue based on Rodgers' evolutionary method of concept analysis.
KEY MESSAGE:
Clarifying this concept is expected to contribute to improving the quality of care and support systems in real life. Mothers raising multiples walk a path where joy and burden overlap in multiple layers. Midwives can walk beside them, not with answers, but with understanding.
