"Uruguay crece contigo": Tthe contribution of midwife obstetricians to early childhood protection and sexual and reproductive health

ABSTRACT
PURPOSE:
To share the experience of the role played by midwife obstetricians within the framework of the Uruguay Crece Contigo (UCC) program, as part of a public policy aimed at the comprehensive protection of vulnerable early childhood.
DISCUSSION:
The Uruguay Crece Contigo program was created in 2012 by the Planning and Budget Office of the Presidency of the Oriental Republic of Uruguay and institutionalized in 2015 as a National Directorate under the Ministry of Social Development (MIDES). It represents a national, intersectoral, and inter-institutional strategy that combines universal interventions with targeted actions. Territorial intervention is implemented through interdisciplinary pairs conducting home visits to families with pregnant women and/or children under four years old, following specific protocols. In this context, midwife obstetricians play a key role by contributing a comprehensive sexual and reproductive health perspective. Their functions include sexual health education, contraceptive counseling, prevention of cervical cancer and sexually transmitted infections, support during voluntary pregnancy termination, monitoring of pregnancy and postpartum care, breastfeeding support, newborn care guidance, and promotion of safe sleep. These activities are framed within a rights-, gender-, and generational-based approach, coordinated with local networks and community actors.
EVIDENCE WHERE RELEVANT:
Interventions are systematically recorded through official state electronic forms and logs, enabling monitoring of implemented actions. Evidence shows that the work of midwife obstetricians strengthens the link between families and health services, promoting timely access and continuity of care.
KEY MESSAGE:
The inclusion of midwife obstetricians in interdisciplinary territorial teams facilitates a comprehensive approach to maternal and child health and reinforces a rights-based framework in supporting families in vulnerable situations.
