Visualizing the role of midwives in planetary health: A proposed graphic model

ABSTRACT
PURPOSE:
This abstract introduces a conceptual graphic model illustrating the role of midwives in advancing planetary health. The model visually represents how midwifery care, grounded in ecological awareness, social justice, and community-centered practice, aligns with planetary health goals and serves as a tool to support education, reflection, and policy transformation. The model is designed to be used in educational settings, professional development, and advocacy efforts to help midwives operationalize planetary health principles in practice.
DISCUSSION:
Midwifery is well-positioned to address the intersection of environmental degradation, reproductive justice, and public health. Yet planetary health remains underrepresented in midwifery education and practice. Drawing from the Planetary Health Education Framework, midwifery core competencies, and global sustainability goals, the model presents six interconnected domains: policy advocacy, ecological sustainability, reproductive justice, social determinants of health, environmental justice, and education and empowerment. These domains emerged through an iterative synthesis of literature, organizational position statements, and interdisciplinary frameworks. The model emphasizes midwives’ unique capacity to reduce healthcare’s environmental footprint, educate patients on exposures, and promote sustainable, equitable systems of care.
EVIDENCE WHERE RELEVANT:
This model is informed by current research linking climate change to adverse maternal outcomes (e.g., heat-related preterm birth, pollution-related complications), and by organizational guidelines from the WHO, ACNM, and ICM calling for greater environmental stewardship in reproductive healthcare. Recent studies also highlight gaps in planetary health content within midwifery education and the urgency of integrating ecological awareness into curricula and clinical practice.
KEY MESSAGE:
Midwives have a critical role in advancing planetary health. This graphic model offers a practical, adaptable framework to guide midwifery education, practice, and policy in responding to the ecological crisis while centering justice and equity.
