ABSTRACT
PURPOSE:
This poster advocates for the mandatory integration of midwife-led care (MLC) on all civil search and rescue missions in the Mediterranean Sea. Midwives are uniquely qualified to provide comprehensive physical, mental, and sexual and reproductive healthcare.
DISCUSSION:
Women crossing the Central Mediterranean - one of the world’s deadliest migration routes - face significant risks, including pregnancy, sexual violence, and lack of access to healthcare. These factors contribute to high rates of maternal and neonatal complications, such as stillbirth, maternal death, and morbidity. The brief time spent aboard rescue vessels presents a crucial opportunity to address these urgent needs. MLC, already implemented by some non-governmental organisations (NGOs), includes care for pregnancy-related complications, survivors of sexual violence, and other gender-specific health concerns. Midwives offer holistic, trauma-informed services tailored to the needs of women and girls, ensuring safe, dignified, and equitable care.
EVIDENCE WHERE RELEVANT:
This approach aligns with international human rights frameworks such as the Universal Declaration of Human Rights and the Sustainable Development Goals, which call for universal access to maternal health services. The Lancet Series on Midwifery and The State of the World's Midwifery 2021 report highlight the essential role of midwives in reducing maternal and neonatal mortality globally. Investment in midwifery education and workforce development is key to improving health outcomes and ensuring access to quality care.
KEY MESSAGE:
To meet international standards for maternal healthcare at sea, this poster proposes:
1. Mandatory inclusion of a trained midwife on every civil rescue ship.
2. Development of standardised training programs.
3. Provision of adequate materials, settings, and resources.
4. Strong collaboration between NGOs for implementation and support.
