Relationship between quality of maternity care and parenting confidence one month postpartum in Japan

ABSTRACT
BACKGROUND:
A woman's childbirth experience and high-quality care from midwives and other maternity professionals can increase maternal self-esteem and confidence in early parenting.
OBJECTIVES:
To examine the relationship between the perceived quality of maternity care or mode of delivery and maternal parenting confidence one month postpartum.
METHODS:
We conducted a prospective survey in Japan. At Time 1, we sent the survey information to all hospitals nationwide where women could give birth. These women were requested to complete an online questionnaire within seven days of giving birth to allow for the assessment of demographics and information during childbirth, depressive symptoms and perceived quality of care (Perceived Care Quality scale; PCQ). Time 2 followed the same cohort at one month postpartum, measuring parenting confidence using the Karitane Parenting Confidence Scale (KPCS). The analysis was restricted to women who delivered at term gestation with a partner. We conducted a multivariate linear regression with the KPCS score as the outcome variable. Research ethics committees of the Graduate School of Medicine, University of Tokyo, approved this study.
RESULTS:
Of the 1,825 women who had completed, 1,689 met the inclusion criteria. Higher PCQ scores were clearly associated with increased KPCS scores at one month postpartum (β = 0.19, p < .001), adjusting for covariates. Compared to spontaneous birth, epidural analgesia was linked to lower parenting confidence (β = -0.05, p < 0.036). Cesarean delivery bore no significant relation to KPCS (β = -0.04, p = 0.090).
CONCLUSIONS:
The quality of care had a lasting effect on maternal confidence during early parenting. Providing respectful and high-quality intrapartum care can play a crucial role in advancing birth outcomes and encouraging maternal confidence in the postpartum period.
