ABSTRACT
PURPOSE:
Intrapartum ultrasound has become established as a highly valuable complementary tool for the objective assessment of labor progression. Its implementation in a tertiary-level hospital represents a paradigm shift in contemporary obstetric care.The active involvement of midwives has been incorporated, integrating them into the use and interpretation of intrapartum ultrasound as part of their clinical practice. To improve diagnostic accuracy, optimize clinical decision-making, and reduce unnecessary interventions. To demonstrate that implementing intrapartum ultrasound in our setting is both feasible and beneficial, with the potential to become standard practice in high-complexity centers.

DISCUSSION:
The parameters assessed include the fetal head position, cervical length and dilation, angle of progression, and head–perineum distance, both in spontaneous and induced labor. We aim to enhance the childbirth experience for patients, increase healthcare team satisfaction, and reduce the number of instrumental deliveries. The participation of midwives has been key to the continuous monitoring of labor progression, supporting their professional development and reinforcing their role within the obstetric team, in close collaboration with medical staff. This integration enhances continuity of care in low- and medium-risk births, reduces fragmentation of care, and maintains the midwife as a key figure throughout the process: women are cared for by the same professionals from pregnancy through to delivery. The use of ultrasound by midwives enables more comprehensive, evidence-based care, and strengthens their role in supporting and making decisions throughout labor.
