ABSTRACT
PURPOSE:
According to the statistics of the National Health Administration in 2023, Taiwan's midwifery delivery rate of 0.22% is the lowest in the world, but it is the highest delivery rate in 16 years since 2009 (0.02%). In 2023, the caesarean section rate 38.41 % reached the highest in all years, which is far from the 10%-15% recommended by the World Health Organization (WHO). Many advanced countries in Europe and America have made great progress in the aspect of "complete system", and the proportion of women who can choose their own midwives to deliver their babies is much higher than that in Taiwan Province.

EVIDENCE WHERE RELEVANT:
The Ministry of Health and Welfare also tried to promote the "Friendly, Diverse and Warm Birth Hospital Pilot Program" in 2014, in which midwives were included in the hospital to serve mothers. At that time, a total of six hospitals participated in the pilot program. The cesarean section rate and episiotomy rate of births delivered by midwives decreased, and the breastfeeding rate increased. The satisfaction of women and their families with this care model reached more than 85%, and the maternal and infant complications were 0%.
KEY MESSAGE:
For me, who was fortunate enough to obtain Taiwan's No. 000001 midwife certificate in 2004, . In recent years, the aging of Taiwan's population and the "feminization of care" have made women's responsibilities more important. However, the caesarean section rate is as high as 38.41%, which is really worrying. Since the restoration of midwifery "education" in 2000, more than 900 midwives have passed the "examination" and obtained certificates. I hope that we can see a more complete medical system soon and implement the policy of making good use of midwives.
