ABSTRACT
PURPOSE:
To present the recently updated Clinical Practice Guideline (CPG) from the Association of Ontario Midwives (AOM) on the prevention and management of postpartum hemorrhage (PPH), with an emphasis on the evidence-based changes and implications for midwifery care.
DISCUSSION:
This session will provide a brief overview of the CPG updating process, followed by an in-depth discussion of the new and revised clinical recommendations. Updates address a range of clinical and systemic factors including revised definitions of PPH, the influence of racism on PPH outcomes, visual and quantitative blood loss measurement, and the latest guidance on both preventive and management interventions, including the use of adjuncts, tranexamic acid, non-pneumatic anti-shock garments, and the incorporation of IV iron therapy in the postpartum period. In addition, knowledge translation tools designed for midwives and clients will be shared.
EVIDENCE WHERE RELEVANT:
The revised guideline is grounded in current evidence and reflects a rigorous review of the literature using GRADE methodology. Evidence has been reviewed by a panel of midwifery experts, ensuring that the literature is interpreted through a midwifery lens and recommendations are relevant to Ontario midwives.
KEY MESSAGE:
This updated CPG supports midwives in providing evidence-informed, client-centered care for the prevention and management of postpartum hemorrhage, while offering practical tools to support midwives in everyday practice.
