Empowering midwives and women through structured family planning counseling: Lessons from two pilot sites in Indonesia

ABSTRACT
BACKGROUND:
Midwives are essential in facilitating informed contraceptive choices among women. As of 2024, over 257,000 midwives serve across in Indonesia, yet challenges remain in ensuring rights-based FP counseling. These include coercion in contraceptive use, limited male participation, and a lack of client awareness about contraceptive options. Balanced counseling is critical for enabling informed decision-making, and the Balanced Counseling Strategy (BCS) with visual counseling cards offers a structured approach to support midwives in providing respectful, women centred FP services.

OBJECTIVES:
To assess the impact of BCS counseling cards on midwives’ confidence and women’s contraceptive decision-making autonomy in two distinct demographic settings: industrial Cikarang (West Java) and urban East Jakarta (DKI Jakarta).

METHODS:
A pilot quasi-experimental study was conducted involving training of 30 midwives in using BCS counseling cards during FP services. Cikarang represents an industrial zone with a high number of female migrant workers, while East Jakarta is a densely populated urban area. Data were collected through pre- and post-intervention self-assessments of midwives and women of reproductive age.

RESULTS:
In Cikarang, 96% of midwives felt that was an increased self-confidence in providing family planning counseling using flash cards, and 97% of women reported making autonomous contraceptive decisions after receiving counselling. In East Jakarta, midwives' confidence reached 97%, and 94% of women independently selected a contraceptive method. These consistent outcomes in different demographic settings highlight the tool's adaptability and effectiveness.

CONCLUSIONS:
BCS counseling cards enhanced both midwives confidence and women's decision-making autonomy. This approach can standardize counseling quality and promote reproductive rights across diverse regions.
