ABSTRACT
PURPOSE:
To describe the case study of a Haitian woman who suffered an eclamptic seizure, emergency c-section with blood donated by local midwife, and then postpartum psychosis.
DISCUSSION:
Midwife Jane Drichta will detail the incredible success story:Elane’s story is one of survival, resilience, and the extraordinary power of midwifery care. It is also a story of innovation, of love, and of a groundbreaking program that is changing maternal healthcare in Haiti.Elane was 38 years old, unmarried, and already a mother of five when she was referred to Kay Manman Yo, Haiti’s first and only high-risk maternal waiting home. She had severe pregnancy complications and was diagnosed with pre-eclampsia. Elane’s condition rapidly worsened. Her blood pressure skyrocketed to 202/100, and she suffered an eclamptic seizure, falling into a 12-hour coma. Even though an emergency C-section was her only chance, there was no blood for a transfusion. One of the Midwives for Haiti staff shared Elane’s O+ blood type, allowing for the urgent surgery. As Elane emerged from anesthesia, a new crisis unfolded—postpartum psychosis. Though there was a hospital psychologist, his interventions proved ineffective. Dealing with both the neurological aftermath of her seizure and the psychological distress of postpartum psychosis, what followed was an innovative, life-saving intervention, developed through collaboration with a perinatal mental health therapist in Seattle and a neurologist in Boston. With no access to high-tech medical solutions, the team crafted a care plan using simple but effective tools—midwifery, nutrition, emotional support, and medical ingenuity.Every aspect of care was designed to address her physical and neurological recovery. This was not just a treatment plan—it was a symphony of science, culture, and care, where each piece depended on the others.
