An integrated midwifery model of continuity of care for pregnant survivors of sexual and gender based violence in DR Congo

ABSTRACT
PURPOSE:
Women previously exposed to sexual violence are at higher risk for adverse pregnancy and obstetric outcomes, risk of re-traumatisation during birth, and for difficulties to attach to their newborns. To suit the complex needs of women and girls who have become pregnant as a result of sexual violence, this presentation demonstrates a person-centred, and transformative model of midwifery care in a low-income and conflict-affected setting with high precedence of sexual violence. The model was developed at the Panzi hospital in eastern DRC and evolved through collaboration between the midwives, the inter-professional team and survivor mentors .
DISCUSSION:
Well-educated and trained midwives working in an enabling environment are key in person-centred sexual and reproductive health care across the life course. This is true also in the most difficult humanitarian and conflict-affected settings
EVIDENCE WHERE RELEVANT:
The model strengthens midwifery skills, professional autonomy, and collaboration, working conditions and satisfaction benefiting all women giving birth at the facility. It cultivates a deeper understanding and competence of the impacts of sexual violence in pregnancy and childbirth and provides continuity of care in all areas of expertise needed for a safe birth experience that empowers rather than re-awakens the initial sexual violence trauma.
Between 2021-2024 625 women have experienced this model of care in Panzi Hospital. In 2024 77% were minors. Indicative results show a decrease in C-section rates from initial 50% in 2020 to 40% in 2024.
KEY MESSAGE:
This midwife led model of care illustrates an opportunity for pregnant survivors of sexual violence to not only survive childbirth but to thrive and transform in the process. It inspires action, utilizes innovative approaches and stresses the need for capacity building and interprofessional collaboration. It presents a strategy in line with Agenda 2030, and the SDGs, to reach those furthest behind that can be scaled up in other similar contexts.
