ABSTRACT
PURPOSE:
The State of the Worlds Midwifery report 2021 identified a more restricted scope of practice of midwives in Europe and the Eastern Mediterranean than in lower-middle income countries. The purpose of this initiative was to provide Midwifery Led care for women with suspected breech presentation from 34 to 36 weeks’ gestation attending the Midwifery Led Unit in Our Lady of Lourdes Maternity Unit, Ireland. This initiative was to optimise the role of our midwives. To achieve this, midwives were trained and deemed competent to use diagnostic ultrasound to confirm fetal presentation suspected at 36 weeks’ gestation onward. Midwives completed a 'Basic Ultrasound Skills Programme'
DISCUSSION:
As midwives, the aims are to confirm presentation of baby after 36 weeks’ gestation following a suspicion of breech presentation, ensure women are counselled to assist physiological natural rotation of breech presentation to head–down presentation, promote continuity of carer, enabling midwives to provide midwifery services across the full continuum of care. Continuity of Midwifery Care is associated with improved clinical outcomes, greater maternal satisfaction, and improved work experiences for midwives. Case Study “At my 38-week appointment, my midwife felt my baby was breech and was able to do a handheld scan, and confirmed it was bum down. I was recommended specific exercises to help encourage baby to turn naturally. I diligently followed her advice and at the next antenatal clinic we discovered the baby turned naturally. I was thrilled that I didn’t require any other interventions, the use of the hand held scan reduced my waiting time and provided me with a continuity of care. My midwives’ guidance and support made a significant impact on my pregnancy. I am truly grateful for the exceptional care she provided”
