Validating a third trimester bedside ultrasound clinical decision support algorithm in rural Uganda

ABSTRACT
BACKGROUND:
Imaging the World Africa (ITWA) a registered NGO supports availability of diagnostic ultrasound services to rural underserved communities in Uganda, Kenya and Malawi. ITWA trains and supports midwives from rural facilities to integrate basic obstetric ultrasound scan in their practice. Although existing evidence does not address or support 3rd trimester Ultrasound in routine ANC, ITWA developed Clinical Decision Support (CDS) bedside algorithms tool to stratify women post 3rd trimester scan for appropriate management
OBJECTIVES:
Validate appropriateness of the CDS tool in the stratifying women that require special care following a 3rd trimester scan
METHODS:
IBR approval was obtained from Uganda, and consent for a prospective study of consenting women/girls attending normal ANC between 28-41 weeks. Midwife sonographers scanned to capture all the anatomy of the pregnancy in 6 sweeps over the abdomen using a portable bedside Lumify Ultrasound systems with a C5-2 transducer
RESULTS:
1307 subjects were scanned at 28-41 weeks gestation and were followed through delivery and up to 42 days. Based on the CDS tool, 85% of scans were assigned to low risk (n=1110), 9% intermediate risk (n=113) and 6% high risk (n=84). Of those assigned to low risk, 97% (n=938) had an uncomplicated birth and 3% (n=27) had a complicated birth. For intermediate risk scans, 79% (n=88) had an uncomplicated birth and 21% (n=25) had a complicated birth. Of high-risk assignments, 56% (n=47) had uncomplicated births and 44% (n=37) had complicated births.
CONCLUSIONS:
The CDS bedside tool validation confirms that 3rd trimester ultrasound has a central role in fetal surveillance. Third trimester scan can identify complication for timely management.
KEY MESSAGE:
· Midwife-sonographers can interpret scans correctly, manage the workload effectively and safely if provided with appropriate support. The can play a critical role in improving access to maternal health intervention in undeserved communities· Third Trimester scan has potential of improving pregnancy outcomes
