Reflective practice for midwives in the United States: An adaptation of the Bass Model of Holistic Reflection

ABSTRACT
PURPOSE:
The purpose of this oral presentation is to share an adaptation of the Bass Model of Holistic Reflection that has recently been used in nurse-midwifery programs in the United States. The presenters will review the Bass Model of Holistic Reflection and the process of increasing personal/professional awareness, explain adaptations to the Bass Model to include the experiences of student midwives in the U.S., and explain how reflective practice can lead to a reduction of early career burnout. Throughout the presentation, examples of the adapted model of reflection applied to specific midwifery experiences will be shared.
DISCUSSION:
The Bass Model of Holistic Reflection used in Australian midwifery programs has been adapted for use by midwives trained in the U.S. The reflections consider the scope of practice for North American midwives and the practice barriers existing in a medical system that has not widely supported midwifery. Midwifery educators analyzed the original model to identify adaptations needed to fit the North American midwifery experience. Comparing the hallmarks of the midwifery model of care to the concepts within the Bass model, reflective grids were established and implemented in several nurse-midwifery education programs. A series of questions was selected to examine self-awareness, emotional regulation, somatic responses, and locus of control during a clinical experience.
EVIDENCE WHERE RELEVANT:
The data on burnout in the midwifery profession demonstrates that support during the early years of practice is crucial to a midwife’s practice longevity. Reflective practice is one tool that increases physical awareness and emotional insight, assisting with professional identity formation.
KEY MESSAGE:
A foundation of holistic reflection provides awareness of the whole self, including how physical, emotional, and environmental perceptions intersect. The challenges during the transition to practice are specific to the practice environment of the midwife. Holistic reflection can support students and new midwives as they transition to practice in the U.S.
