ABSTRACT
BACKGROUND:
Postpartum haemorrhage (PPH) remains a leading cause of maternal mortality globally requiring prompt and effective management to prevent severe outcomes. The International Confederation of Midwives (ICM) midwifery models of care advocates for evidence-based care to address critical maternal health challenges. The E-MOTIVE bundle, developed by the World Health Organization (WHO), offers a structured, evidence informed approach for the early detection and management of PPH.
OBJECTIVES:
The scoping review aimed to map and synthesize evidence factors influencing the implementation of the E-MOTIVE bundle for early detection and management of postpartum hemorrhage (PPH) emergencies by obstetric team globally.
METHODS:
This scoping review was guided by the adapted version of Arksey and O’Malley methodological framework (2005). We searched the following databases from 2015 to 2025: PubMed, SCOPUS, Web of Science, EBSCOhost (CINAHL, MEDLINE and Academic search): and ProQuest central. The quality of the included studies was appraised using mixed methods appraisal tool (MMAT) version 2018. Content analysis and synthesis of the findings were conducted using Nvivo 12
RESULTS:
4 major themes were identified: 1. E-motive bundle improves timely identification of PPH and promoting a standardized, 2. Domains of E-motive bundle: readiness, recognition and prevention, response, and systems learning 3. Barriers to implementation first responders not always skilful. Underestimation of blood loss. 4. Key enablers of successful implementation including midwife-led training, team-based strategies, Partnership for Maternal Safety, Women-centred approach, adoption of standardized policies in facilities.
CONCLUSIONS:
The E-MOTIVE bundle has demonstrated potential to transform PPH response and strengthen maternal health systems when effectively implemented.
KEY MESSAGE:
Implementing the E-MOTIVE bundle enhances midwife-led, evidence-based, and timely management of postpartum haemorrhage, supporting the International Confederation of Midwives (ICM) midwifery model of care by promoting quality, respectful, and woman-centered maternal health services.
